Touchstone is delivering three structured education sessions in partnership with Leeds Community Healthcare, QISMET and Sheffield University’s School of Health and Related Research. The sessions will run for adults across Leeds living with Long Term Health Conditions. These will be co-designed to support individuals to self manage a range of Long Term Health Conditions including diabetes, COPD (chronic obstructive pulmonary disease), stroke, depression, heart disease, etc. This list is not exhaustive and includes any Long Term Condition.
This self management programme is aimed at adults (over the age of 18) who are within 12 months of having being diagnosed with a long term condition or who have not accessed any structured education previously. The programme will be delivered via a world café style approach and will deliver the following information:
(1) Lifestyle factors- smoking, alcohol, weight, physical activity and healthy eating
(2) Emotional health and wellbeing
(3) Adapting to diagnosis/assessing impact on their life
(4) What to expect from health care professionals
(5) Goal setting
(6) Support networks: digital and local community
(7) Practical and financial support
(8) Signposting to further services and information
The programme aims to empower adults living with a Long Term Condition by enabling them to develop the necessary knowledge, skills and confidence to self manage their condition effectively and know how and where to access follow on support.
Sessions will run throughout September, October and November in Leeds city Centre.
