Touchstone, in partnership with Leeds Community Healthcare, QISMET and Sheffield’s School of Health and Related Research have been successful in securing funding for a 6 month project to run structured education sessions to support people living with Long Term health conditions in Leeds.
Sessions are aimed at adults (18+) who are within 12months of having been diagnosed with a Long Term Condition, or who haven’t accessed any structured education before. We will be running groups throughout September, October and November in Leeds city centre.
Our interactive structured education will be co-designed to support individuals to self manage a range of conditions including diabetes, COPD (chronic obstructive pulmonary disease) and respiratory diseases, or any other Long Term Health Condition.
Participants will have the opportunity to learn about different health factors that may impact on their conditions and will be supported by trained staff and volunteers to set goals and make positive changes in their lives. There will also be further information about wider support available in Leeds.
The aim is to empower individuals living with Long Term Conditions and enable them to develop knowledge, confidence and skills around managing their condition, and to know how and where to access further support.
