This page is about what information the Coproduction team will collect from you, and how we will use it.  

Our records are completely separate from the Leeds Mental Wellbeing Service patient information system, which has details about your therapy and treatment. First contact 

WHAT: To start with, we will collect basic personal details so that we can contact you: your name, email address and phone number. We’ll also check how you prefer to be contacted and what we need to do to get in touch with you, for example – do we need to post information in large print, or arrange an interpreter. 

WHERE: We will store this information in a secure electronic list. Only members of the small Coproduction team can access your information. Staff have lots of training about using personal data safely and respectfully. 

WHY: We will only use this information to contact you about the Coproduction Network. If we don’t hear from you after six months, or if you are not interested in Coproduction, we will delete all the information we have about you. 

Benefits:  

  • We will use your details to share more information with you about the coproduction network 
  • We will delete your information if you aren’t interested in getting involved, or we don’t hear from you for six months
  • We will respect how you prefer to be contacted and any communication needs you may have 

Risks:  

  • Staff within the small Coproduction team will be able to access the information you share with us, and they have lots of training about using personal data safely and respectfully. 

Next steps: Joining the Coproduction Network 

You’ll have a conversation with a member of our team who will explain more about the Coproduction Network and find out if you would like to be involved.  

If you decide you would like to join, we will collect a little more information: 

WHAT: A member of our team will collect some information about your interests, passions, and skills, as well as your experience of using mental health services. They will also ask if there are any adaptions we can make to help you be involved in Coproduction projects and activities. We will also ask for your date of birth, postcode and the details of someone who we can contact in an emergency, in case we ever need these for safeguarding. 

WHERE: We will store this information in our secure electronic Membership Database. 

WHY: We will use this information to contact you about Coproduction activities that match up with your interests. We will use this database to keep a record of which projects you are involved in, so that we only contact the right people about the right things. If we ever have concerns about your safety, we may use your information for safeguarding purposes. You can read more about this in our safeguarding statement. 

Diversity and Equal Opportunities Monitoring 

If you decide to be part of the Coproduction Network, we will ask you to complete a Diversity Monitoring Form. It is your choice if you want to complete this form or not. You can also choose to only complete parts of the form if you prefer. 

WHAT: We use this form to collect information about your identity, for example – your gender identity, your religion, your ethnic background. 

WHERE: We store this information in a secure database on our secure server alongside your personal details.  Your data will be collected using Google Forms.  Once it is transferred to our database it will be permanently removed from Google. 

WHY: We only use this information to get a general picture of how well the Coproduction Network is including different groups of people. For example – we can use this information to see how many people in the Network have a disability, or how many are Sikh. It’s important for us to know if certain groups are not well represented in the Network so that we can make improvements. 

Updating your information 

If any of the information you give us changes, for example if you change address or want to change your emergency contact details, please get in touch to let us know. We will then update the information we hold about you in our Membership Database. 

Viewing your information 

You have a right to view the information we hold about you. If you would like to see this information, please get in touch and we will arrange to share it with you. 

Deleting your information 

You have a right for any information we hold about you to be deleted. If you would like any of the information we hold about you to be deleted, please get in touch to let us know and it will be deleted within one month of us receiving the request in line with GDPR legislation. If you choose to leave the Coproduction Network, we will delete your data after twelve months.
