Who We Are:

Wakefield Community Enablement Team (CET) is a community-based support service which supports individuals with complex mental health needs to live independently within their own communities. We are funded by NHS West Yorkshire Integrated Care Board.

Who We Support:

We support individuals who:

  • Are Aged 16+
  • Have complex mental health needs.
  • Are at risk of a deterioration of mental health, tenancy related issues, (re)admission to hospital or (re)offending.

We are a non-clinical service and are unable to support someone if:

  • They require acute mental health support
  • They require crisis support
  • We cannot provide one-to-one support safely

How We Work:

We are a team of experienced Mental Health Outreach Workers who provide face-to-face, person-centred support sessions at Service Users’ homes or out in the community.

At the start of service each Service User is assigned a designated Outreach Worker. They and their worker identify some recovery-focused goals to work towards and coproduce a support plan. Support focuses on working through this plan step-by-step, at a pace which suits the individual. Sessions are weekly to start with, then move fortnightly, then monthly, as support progresses.

Goals for support might include:

  • Maximising independence and increasing confidence
  • Reducing isolation and engaging in local communities
  • Improving life skills and financial management
  • Retaining home and tenancy
  • Using coping strategies to improve and manage wellbeing

Referrals

As of the 1st of June, we are closed for referrals. Please note that we do not hold a waiting list and will not accept referrals sent at this time.  We aim to reopen by mid-July depending on service capacity. Please refer back to this webpage for updates.

Wakefield Community Enablement team brings together all my experience and passion. I have over 30 years’ experience working in various roles both within the private and voluntary sector across West Yorkshire. Throughout these years the thread of mental health has been consistent and remains my passion. I have seen the massive difference tailored person-centred support can make to people’s lives. Personalised support gives greater flexibility and allows individuals to mould their own support based on what matters to them. Recognizing an individual’s skills, talents and strengths is at the centre of the support Wakefield CET provides.

I have now been working at Touchstone for 19 years in various roles. Every day brings new challenges but it never fails to touch me when I hear about the amazing achievements of the individuals we support. This is what motivated me right back at the start in 1990. Making a difference and being part of someone’s inspirational journey is at the heart of what we do.
