The Wakefield Community Enablement Team is a brand new community-based support service which will support individuals with multiple and complex mental health needs to enable you to live independently within your own community. This service is funded by NHS West Yorkshire Integrated Care Board.

The service is for individuals living in Wakefield and surrounding areas that meet the following:

  • Aged 16+
  • Have complex mental and emotional health needs.
  • Need support with accessing community-based activities.
  • Who may be at risk of a deterioration of mental health, tenancy related issues, re-admission to hospital or risk of re-offending.

What support do we offer?

The Community Enablement Team has a team of experienced Mental Health Outreach workers who will provide a strengths-based offer of one-to-one support sessions. Service users will be allocated a designated outreach worker, who will identify outcomes with you which will be person-centered and recovery-focused.

We will work together to develop a plan to focus on the following areas:

  • Reduce reliance on crisis services
  • Maximising independence & increasing confidence
  • Support to liaise with other professionals attend meetings and appointments
  • Keeping safe and managing risk
  • Promote your strengths , skills and coping strategies to improve and manage wellbeing
  • Reduce isolation and engage in your local community .
  • Increase independence, choice and control.
  • Develop skills and access to education , training and employment .
  • Develop and increase life skills & financial management.
  • Support to retain your tenancy / home as independently as possible .

We are a non-clinical team and we may not be the right service for you if:

  • You need crisis support.
  • You need acute mental health support.
  • We are unable to offer 1-1 support safely.

I am so excited to be part of this brand new Community based Support service to Wakefield and its districts. It really brings together all of my passions and experience.

I have over 30 years’ experience working in various roles both within the private and voluntary sector across West Yorkshire. Throughout these years the thread of mental health has been consistent and remains my passion. I have seen first hand the difference delivering tailored person-centred support can make. Personalised support gives greater flexibility to the individual to mould their own support in a way that matters to them which is what makes a real difference. Recognizing an individual’s skills, talents and strengths is right at the centre of what we aim to focus on together with the people we support .

I have now been working at Touchstone for 19 years in various roles, every day brings new challenges but it never fails to touch me when I hear about the amazing achievements from the individuals we support. This is what motivated me right back at the start in 1990, making a difference and being part of someone’s inspirational journey no matter how big or small remains what matters at the heart of what we do.
