Supporting service users with complex social and health needs including those in Crisis. More intensive and tailor made coproduced support to sustain wellbeing and promote recovery in the community.
Touchstone is a partner in a Leeds city-wide initiative that provides supported accommodation for individuals and families with complex needs, including those who are homeless or at risk of homelessness. Beacon was launched in 2017 and is delivered by a consortium including Turning Lives Around, Touchstone and Foundation.
CST is an intensive mental health support service using an assertive outreach approach. CST works with service users who have complex mental health needs-service users referred must have a mental health condition that falls within mental health clusters 4-8, or 11-17.
CRISS provides Peer Support to people that have been referred to CRISS, working in partnership with the NHS, to support those who are experiencing a mental health crisis as an alternative to hospital. Service users are referred to CRISS if they need urgent mental health support and interventions to help their recovery at home.
If you or somebody you know are struggling with thoughts of feeling down, worried, overwhelmed, or may have thoughts of self-harm and suicide, Here For You have local teams available every night of the year from 6pm until midnight across Leeds, Kirklees, and Wakefield.
The service offers free, safe, and confidential face-to-face support for anybody aged 16 years or older in Leeds and Wakefield, or 18 years or older in Kirklees.
1-to-1 support is available, if you have something you feel you need to talk about privately, or if you prefer support around other people, we offer a social space for up to 6 people each evening.
This service helps people with a personal budget in Kirklees to find and arrange the kind of support services they want. All our peer brokers are people with direct experience of living with mental health difficulties (including dementia), and of using mental health services. They are trained and supported to help you work out what kind of support you want, to live the kind of life you want.
Rotherham Safe Space supports anyone experiencing a mental health crisis in Rotherham and the surrounding areas. It provides a safe place during the weekend evenings designed for people in crisis to go for support and to prevent avoidable attendances at A&E.
The Wakefield Community Enablement Team is a community-based support service which will support individuals with multiple and complex mental health needs to enable you to live independently within your own community. This service is funded by NHS West Yorkshire Integrated Care Board.
