Evaluation of community diabetes course

Touchstone to run LTC Self Management Sessions

Touchstone, in partnership with Leeds Community Healthcare, QISMET and Sheffield’s School of Health and Related Research have been successful in securing funding for a 6 month project to run structured education sessions to support people living with Long Term health conditions in Leeds. Sessions are aimed at adults (18+) who are within 12months of having been diagnosed with a Long Term Condition, or who haven’t accessed any structured education before.

Young leaders go international
